FMEA report + proving trials report

Establishes, by failure mode analysis across the whole DP system and by the proving trials that demonstrate it, whether the worst case failure found matches the worst case failure the design intended. Those two are different things, and the distance between them is the finding.

In scope

  • Failure mode and effects analysis across the whole DP system — power generation and distribution, thrusters and their control, the DP control system, position reference systems, sensors and the vessel management network.
  • Determination of the Worst Case Failure (WCF) and its comparison against the declared Worst Case Failure Design Intent (WCFDI), for every operational configuration.
  • Analysis structured over performance, protection and detection — the three elements of any redundancy-based fault-tolerant system.
  • Proving trials on board, demonstrating in practice what the analysis asserts on paper.
  • Revision of the FMEA against the trial results.

Not in scope

  • Engineering of the corrective action for findings raised. We identify and categorise; the remediation design belongs to the yard or the OEM.
  • Issue of capability plots. These are a naval architecture deliverable and we consume them rather than produce them.
  • Repair, adjustment or commissioning of equipment during attendance.

  1. FMEA report, structured to IMCA M 166 §2.4.1.
  2. Proving trials report, recording each test, its acceptance criterion and the observed result.
  3. Revised FMEA, incorporating what the trials established.
  4. Detailed Service Report — consultative, frank and confidential to the owner. Every finding also appears in the formal report; the difference is depth of advice, never suppression.
  5. Crew summary — what the analysis changed for the people who stand the watch.
